I have looked around long and hard for a page which nicely describes the Arduino Mega connector/hole locations. They all suggest using the Eagle files for the card on the arduino site. I did that and created the following mark-up:

A few notes
- Dimensions are in 1/1000's of an inch. For instance, the hole at (600,100) is at 0.6 inches to the right and up 0.1 inches.
- Based on what I have read in various places, I believe that the pin at (950, 2000) really should be at (940, 2000). Some things I have read make me think it's really at (935).
- You may have to do some math if your CAD program puts 0,0 at the top left (as ExpressPCB does).
- You may have to do even more math if your shield is offset (bigger than the shield above). For me, I put my shield at the corner and then move it later inside the board footprint.
- This layout should work fine for other Arduinos, such as the UNO although I haven't included the mounting hole bombsights for those cards.
- This may be done wrong. Check to make sure it is right. I print out a 1:1 and put it on top of the Mega processor board to check for stupid errors.
